Through our Website, you can connect with licensed clinicians to receive certain health care services via telehealth (the "Services").

By accessing the Website, you agree that you have read, understood, and agreed to be bound by all of these Terms of Use. IF YOU DO NOT AGREE WITH ALL OF THESE TERMS OF USE, YOU ARE EXPRESSLY PROHIBITED FROM USING THE SITE AND MUST DISCONTINUE USE IMMEDIATELY. We reserve the right to make changes to these Terms at any time, and your continued use of the Website signifies your acceptance of those changes.

2. Eligibility and User Representations

Age Requirement. The Website is intended for users who are at least 18 years old. Persons under the age of 18 are not permitted to use or register for the Site.

4. Subscriptions, Billing, and Payments

Subscription Services. Revive offers certain services on a subscription basis utilizing monthly (every four weeks) billing. Subscriptions include chosen treatment options, periodic follow-up consultations, and clinical messaging. Subscriptions automatically renew until canceled.

Cancellations. You may cancel your Subscription at any time prior to the next billing date to avoid charges for the next cycle. Cancellations can be made by emailing hello@revivewithme.com or texting +1 (404) 948-6677.

Payments. Revive does not accept insurance and will not submit claims to your insurance provider. You are responsible for the payment of all Services. We accept credit and debit cards, and you agree to provide current and accurate payment information.

Pricing and Returns. We may change prices at any time. All sales are final and no refund will be issued. We also reserve the right to limit or cancel quantities purchased per person, household, or order.

Visiting the Site, sending emails, and completing online forms constitute electronic communications. Revive will communicate with you by telephone, text (such as via Twilio for customer service, invoicing, reminders, and shipping information), email, or by posting notices on the Website. 11. Dispute Resolution and Binding Arbitration

These Terms of Use are governed by and construed in accordance with the laws of the State of Georgia and controlling US Federal Law. All disputes, controversies, or claims arising out of this Agreement will be resolved through mandatory binding arbitration conducted in Atlanta, Georgia, in accordance with AAA rules. You agree to give up your right to go to court, and any dispute must be resolved through individual arbitration, not as a class action.

Severability. If any provision of this Agreement is found to be invalid or unenforceable, it shall be deemed omitted, and the remaining provisions will continue in full force and effect.

Entire Agreement. These Terms constitute the entire agreement between you and Revive regarding your use of the Website.

Force Majeure. Revive's performance is subject to interruption due to causes beyond its reasonable control, including acts of God, war, pandemics, power failures, or labor disputes.
